It is recommended that you use this document as an action tracking tool throughout the year to support ongoing and future conversations.
For Non-Clinical Staff
This section is your action tracker for the next year. It is a live document and can be edited by you and your Line Manager. There is no need to record everything that was discussed in the conversation. It is designed to capture brief notes and actions from your ARC. The responsibility for completing this section lies with the individual staff member during and/or after the conversation. Both you and your Line Manager should use this section to support regular conversations about work and development goals.For Clinical Staff with an Internal Clinical Lead
The Clinical Lead should sign off to confirm that they have seen/reviewed the ARC meeting summary & objectives (in case they were unable to attend the ARC meeting). *‘Clinical Lead’ is usually the Clinical Lead at the NHS Trust. Because you are a Clinical Academic, please ensure that this section covers both the Imperial ARC process as well as your Trust’s process. The individual, Line Manager, and Clinical Lead should be able to access this ARC Form to support regular conversations about work and development goals, as well as to aid planning.For Clinical Staff with an External Clinical Lead
The External Clinical Lead should confirm that they have seen/reviewed the ARC meeting summary & objectives (in case they were unable to attend the ARC meeting). *‘Clinical Lead’ is usually the Clinical Lead at the NHS Trust. Because you are a Clinical Academic, please ensure that this section covers both the Imperial ARC process as well as your Trust’s process. The individual and Line Manager should be able to access this ARC Form to support regular conversations about work and development goals, in consultation with your external clinical lead, as well as to aid planning.
Important: As the Clinical Lead is external to Imperial, they do not have access to the ARC platform.
Please coordinate with your Line Manager to involve them in ARC meetings and objective-setting (e.g. via email).
